What to bring to the dentist and what to expect on your first visit?

Our administrative staff is ready to help you with any questions about scheduling, financial policy and insurance; we’ll make that part of the process as simple as possible.
We are dedicated to providing you with gentle care and high quality services in a comfortable but professional setting so that you can achieve and maintain a beautiful smile and also harmonious overall health.
Your First Visit includes a wellness scan which provides a 3D image of your teeth we use to educate and diagnose treatment.
Also includes:
- comprehensive exam of entire mouth and teeth
- low radiation digital x-rays
- Oral Cancer screening
This section offers information from the administrative side of Roots on Whyte Dental, including what to bring to the dentist and what to expect on your first visit.
Your Questions & Concerns
It is our goal to attend to your needs and we will come up with a treatment plan that is tailored to you. In order for us to do so, we will need you to bring your completed new patient forms to your first visit. If you have not had a chance to complete the forms, please come at least 15 minutes prior to your appointment to complete them.
During your initial exam, we will review your dental, medical and health history and take x-rays of your teeth and the bone structure around them. Then the dentist will go over the x-rays with you, perform a thorough and comprehensive exam of your face, teeth and mouth, provide you with an individualized treatment plan and answer any questions you may have. If you have also booked for your cleaning, then one of our hygienists will clean and polish your teeth, go over brushing and flossing techniques with you and also discuss how you can better maintain your teeth and gum health.
Before your first visit with us, take a few minutes to think over what you’d like to discuss with us, so that your concerns are at the forefront of your mind when the time comes for your appointment.
Please assist us by providing the following information at the time of your consultation. This will help us to help you process any claims in a timely manner.
Medical & Dental History
In order to provide you with the most appropriate care for your dental health needs, we’ll need to know about your dental and medical history.
This includes everything from the medications you’re taking (that may be unrelated to your dental care) to dental restorations and procedures that you’ve undergone in the past, and x-rays.
If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office in Edmonton. However, any new presenting conditions and emergency treatments often require new radiographs to better help Dr. William Yoo, Dr. Terence Clark & Dr. Ruby Aujla with your diagnosis. If additional films are necessary, they can be taken at our facility in Edmonton with our state-of-the-art radiograph equipment.
These details will help us develop a dental healthcare plan that is customized for your unique requirements.
With your permission, your previous dental office will be able to forward us your existing files and information. That said, we will ask you to fill out a new Patient History Form upon your arrival for your first appointment with us, to ensure all the details we have are all up to date.
Insurance Information
After each appointment you have with us, our administrative team will submit insurances claim forms to your insurance company on your behalf.
In order to do this, we will need you to supply up-to-date details regarding your insurance plan, including the insurer name and mailing address, the group/policy/contract number, and your ID/certificate number.
Please be sure to have this information handy for your first appointment with us.
